I would think that would be pretty idiosyncratic for each library, depending on your staffing availability, number of subscriptions, and vendor.

For instance, we're a small academic library and down to under 600 such subscriptions, which is few enough that our annual renewal routine with EBSCO includes:
check the usage data for every online or online+print title and consult with appropriate other library staff for possible cancellations if low usage
check the historical price data for all titles, and bring any significant increases to the attention of appropriate library staff (subject librarian especially)
for print only titles, check the appropriate abstracting service (mostly EBSCOHost databases) and our OpenURL service usage data for evidence of high or low interest in that title

We start checking all that data over the summer, have meetings in Sept to discuss possible cancellations, and submit the actual final renewal (online) at the end of September.

I imagine libraries with a much larger list can't afford the time to hand-check all that data for every title every year.
