Hello Kaia.

I was a bit surprised to see this question because my assumption is that most of us use a subscription agent with a web-based or automated claim system of some sort, and therefore we do not send individual claim letters ourselves.  However, I could be making an erroneous assumption.

My suggestion is that if you have to do such letters yourself, just include basic but complete information in a straightforward manner.  Reference your account number with the publisher in bold lettering.  State the title of the journal, the volume and issue(s) that you need, the reasons (missing or skipped, late, damaged, etc.), and any prior history of claiming the same item.

Examples of the latter might be "This my 2nd claim.  I first claimed this issue by telephone on ________ [give the exact date] in conversation with ___________ [give the name of the person with whom you spoke] and am still waiting to receive it." or "I claimed this issue in my letter of __________ [again, provide exact date] with no results so far."

For your sake, I hope that you will be able to work with a subscription agent soon, since claiming can be time-consuming even with the help of whatever system the agent provides.  Without it, I imagine that the work could feel totally overwhelming, depending on the number of subscriptions you have and the number of publishers with whom you have to maintain separate contacts.
