If you took the periodical survey I sent in the last couple of hours, you may have experienced problems with being able to check more than one response for questions saying "check all that apply".  If so, I have contacted Survey Monkey, and am waiting for a solution.

Sorry this happened!  I'll let you know when the problem has been resolved.
