This is one of those that I would be looking at the mastehead, an editorial note and the scope of the articles in both titles to see if there had been a change in scope of the journal that requires a title change.  Care does not necessarily mean the same thing as medicine.

Hi, all- would those experienced catalogers out there consider this a major title change, from _American journal of hospice & palliative care_ to _American journal of hospice & palliative medicine_?  OCLC #21269606 considers it a minor change, while several OCLC #s, including 57519500, consider it a major change.
