I would agree with what has been said about the change of the last word from "care" to "medicine" being minor change, except that this change was intentional by the publisher (see its editorial on Mar./Apr. 2004 issue: http://ajh.sagepub.com/cgi/reprint/21/2/82)
It clearly stated "To mark the rise of end-of-life care as a specialty, we've upgraded the journal's title as well. Beginning with this issue, the
American Journal of Hospice and Palliative Care graduates to the American Journal of Hospice and Palliative Medicine."

Does the publisher's intention not trump our minor change rule?

This should be considered a minor change, as recorded on OCLC
#57519500.  The changed word is beyond the first five words and was
considered not to have changed the meaning of the title.  The record has
CONSER and NSDP authentications in field 042. The last library to update
the record was the National Library of Medicine.

#57519500 has been reported to OCLC for deletion according to information
in field 936 of that record.

I believe this is a good example of a minor change where the user is best
served by keeping all the bibliographic information on one record.  Many
users might not even notice the difference between the two titles.
