Does anyone have information concerning the Inc. issue that is labeled on
the spine 1996, No.4 and Vol.18, No.17 on the table of contents page?  Each
page has 1996, No.4 on the bottom.  Vol.18, No.17 can be found on the
table of contents page, publishers information column and with pub
pattern information.  There is no date on this issue.  I am concerned as
to how this should be checked in.

Also we did not receive Vol.18, No.15 and I was wondering if anyone else
had the same problem.
