the FRONT of the issue stating that it is a replacement and why.

  ***Most definitely
  replace the entire issue!  It would cause us no end of problems to
  receive a 32 page insert.  Put a removable sticker (of contrasting color
  so as to be noticed) on the cover of the issue clearly stating that it
  is a replacement for a defective issue.  Most check in staff are likely
  to notice information on the cover.  A letter or other enclose might be
  lost.
