I have mixed feelings on the "death during generation" issue.  On one
hand, as you say, it is part of the mystique of the game. However, I had
a player once say, "I just spent the last half hour or so making roll
after roll and now I'm dead?!?  I'm done." It also wastes time during
the start if it happens because now you're waiting on one character
while everyone else is just about finished.

Personally, I'd be happier if it wasn't there.
