I suspect very much that this is why an early shared "house rule" was
what became the "optional survival rule" which is now essentially the
default, with "die, loser!" being the defacto option. I also suspect
very much that variations on the theme were among the first "house
rules" that practically any gaming group made up, just to avoid the
situation you describe here.
